At the point of booking the appointment with your surgery, you will be asked whether you agree to share your patient record so that the local clinicians at the GP Hub are able to access your record and also share relevant information back to your Surgery. This is necessary so that the appropriate notes and referrals can be passed on to your surgery for follow-up actions.
